How the secure pathway will work

  1. Review PathWell's public program information and confirm whether a structured adult outpatient setting may be relevant.
  2. Use the secure referral portal when it is available. The public website will link to the portal but will not collect or store referral details.
  3. The appropriate PathWell team reviews the referral through the secure system and follows the approved response process.

Do not email, text, or enter patient names, dates of birth, symptoms, diagnoses, medications, insurance/member IDs, appointment details, referral details, or other private health information on this public website.

This public website does not accept referral submissions, patient documents, uploads, or referral-status requests.
