This website shares program information and helps you contact PathWell about a referral.
Referral information
When online submissions are available, our referral form sends the information you provide to PathWell’s referral inbox. Our team uses it to review your request and contact you about next steps. Please provide only the requested details. Do not include Social Security numbers, payment information, or medical records.
A referral does not confirm admission or an appointment. Do not use the referral form for emergencies.
You may optionally attach photos of the front and back of your insurance or Medicaid card. These photos travel with the referral email and are retained in PathWell’s email system for referral review. Card photos are not required to request contact.
General email
Email is for general business questions. Do not send patient information, medical records, or insurance details by ordinary email. Use the referral page or call PathWell for help with a referral.

About this website
The website does not use advertising trackers or provide online patient accounts. Hosting services may process routine technical information, such as an IP address, when a page loads. Referral details are not saved in your browser by this website.
