Where IOP fits

An intensive outpatient program is more support than a weekly appointment, without an overnight stay. You live at home and come in for scheduled group mornings.

It may be worth asking about if

  • weekly appointments don't feel like enough right now
  • symptoms keep getting in the way of work, home or relationships, even with the support you already have
  • you want more structure but need to keep living at home

Weekly outpatient care

Regular appointments, often once a week.

Intensive outpatient (IOP)

Structured group care on several days a week, while living at home.

PathWell: 3 mornings a week

More intensive care

Programs that meet for most of the day, or include an overnight stay.

A simplified view. Researchers have described intensive outpatient programs as an option for people who need more than one session a week but less than inpatient or day-clinic care (Driessen and colleagues, 2019). The clinical assessment decides which level fits you.

If you're the one who's worried

You can learn how the program works before you bring it up, and you don't need the perfect words.

  1. Learn first

Read through how it starts and what it costs. For general questions, call our front desk at 703-659-4686. You'll know what you're suggesting before you suggest it.

  2. Share what you've noticed

Be specific and kind: what you've seen, and that you care. Try not to diagnose, and don't push for a decision on the spot.

  3. Make the first step easier

Offer to help them choose a time for the free 15-minute call, or with rides on group mornings.

Worried about their safety right now? Call or text 988 (Suicide & Crisis Lifeline), or chat at 988lifeline.org. You can contact 988 about someone else, too. If someone is in immediate danger, call 911.

What changed in a similar program

In a study of 66 adults in an intensive outpatient program, the group's average depression and anxiety scores were lower at the end than at the start.

That program met three days a week for three hours a day, a schedule comparable to PathWell's group mornings.

Depression symptomsCUDOS questionnaire, 0–64

Depression symptoms: group average 29.49 at the start and 12.77 at the end, on a scale from 0 to 64 where lower means fewer symptoms.

Anxiety symptomsCUXOS questionnaire, 0–80

Anxiety symptoms: group average 33.65 at the start and 17.80 at the end, on a scale from 0 to 80 where lower means fewer symptoms.

Hollow dot: group average at the start. Filled dot: group average at the end. Lower scores mean fewer symptoms.

Step 1 of 415 minutesBy phoneFree

A free call with our therapist

Choose a time that suits you. Our therapist will listen to what's been going on, explain how the program works and answer your questions.

You don't need to know what kind of care you need. Use the call to ask questions and hear what the next step could involve.

Good things to ask about

  • How group mornings work
  • What the assessment involves
  • Costs, insurance or Medicaid
  • How family can be involved
Step 2 of 445–60 minutesWith a clinicianFree to you

A clinical assessment

Your clinical assessment is free to you. We’ll talk through your needs and help you decide on the next step.

We'll talk about your symptoms, care you've tried, the support around you and your safety.

What it's for

  • Understanding what's been hard and what has helped before
  • Checking whether IOP is the right level of care
  • Explaining the recommendation in plain terms
Step 3 of 4A shared decision

Decide together

You'll go over the clinician's recommendation and ask anything you need to. There are two ways this can go.

If IOP fits

Confirm how you'll pay (your exact insurance or Medicaid plan, or self-pay), any approval your plan requires, your attendance plan and a start date.

Then: group mornings begin

If something else fits better

The clinician will explain why and talk through next steps. That might be routine outpatient care or more intensive support.

With Virginia Medicaid

Your exact plan, eligibility, clinical fit and any prior approval need to be checked. A plan's name alone does not confirm PathWell participation or coverage.

Questions to ask

  • Does PathWell participate in my exact plan?
  • Does my IOP care need prior approval?
  • What might I owe, if anything?

If you pay yourself

$250 covers the entire scheduled treatment day, 9:00 AM–12:30 PM: three treatment hours and 30 minutes of breaks. Three treatment days cost $750. It's a daily fee, not a per-group or per-hour charge.

Questions to ask

  • How many treatment days are recommended for me?
  • Could a need-based scholarship help? It's considered case by case and isn't guaranteed.

Questions people ask

Questions about getting started.

What is an intensive outpatient program (IOP)?

Structured mental health treatment without an overnight stay. You attend scheduled group mornings and keep living at home. A clinical assessment decides whether this level of care fits.

Do I have to commit to anything on the call?

The free 15-minute call is an introduction and a chance to ask questions. A clinical assessment is the next step in considering whether IOP fits.

Can I keep working or going to school?

Compare your commitments and travel time with the schedule: Monday, Tuesday and Thursday, 9:00 AM–12:30 PM. Ask about attendance expectations before you plan a start date.

Can family be involved?
